Qualifications

Academic nurse with a completed scientific degree at the master's level or higher in nursing science or advanced practice nursing. Several years of professional experience in caring for patients with dementia and/or delirium, ideally evidenced by the additional training ZERCUR Geriatrie basic course. In-depth knowledge of current guidelines and evidence-based treatment concepts for dementia and delirium. Experience in interdisciplinary collaboration and in advising and training professional groups. Strong ability to coordinate complex care processes. High level of communication and moderation skills in interaction with diverse professional groups and family members. Ability to analyze care processes and develop improvement measures.

Key Responsibilities:

  • Operational management and coordination of the consulting service for dementia and delirium.
  • Planning and controlling resources, including staff scheduling and process workflows within the consulting service.
  • Act as a liaison and primary contact for nursing management and external partners regarding inquiries.
  • Contribute to the development of quality measures and evaluate the consulting activities.
  • Organize and conduct training sessions for various professional groups, particularly in nursing and therapy.
  • Participate in the design of in-house training programs through the IBBZ.
  • Engage in strategic projects, such as implementing new concepts or addressing ethical issues.
  • Monitor and analyze defined quality metrics.

Clinical Duties:

  • Carry out consulting activities, including:
    • Support in the nursing diagnostic assessment of patients with dementia, cognitive impairments, or delirium risk.
    • Assist in planning and implementing individualized treatment and care strategies (non-pharmacological measures, medication recommendations in collaboration with the medical team).
  • Support complex care situations and assist with challenging behaviors.
  • Advise and train family members on managing dementia and delirium.
  • Provide case-based consultations, discussions, analyses, and supervision to support teams in daily practice.
  • Actively participate in the development of intersectoral care for supported patients in the consulting service alongside CHNs at the PORT-Zentrum City location.
