What to Expect:

This position offers a hybrid work environment, allowing you to engage with clients in their homes within San Francisco County, at our office, or from the comfort of your own home.

Your Role:

We seek individuals who are passionate about advocating for individuals over the age of 3 with developmental disabilities and their families. Your responsibilities will include conducting assessments, developing and implementing Individual Program Plans, and ensuring meticulous documentation. You will handle a diverse caseload that demands the expertise of a qualified social worker, including innovative planning and resource acquisition.

You will also engage in ongoing social assessments and comprehensive case planning, gathering vital data for interdisciplinary teams. Your role will involve designing, monitoring, and adjusting program plans, analyzing data to identify barriers, initiating and terminating regional center funding, and ensuring the timely delivery of services. You will advocate for individuals and their families, ensuring they receive all entitled services and actively participate in utilizing available resources.

Additionally, you may provide outreach services as assigned and contribute to internal and community committees and task forces.
