Role overview
The Clinical Guide, Population Health RN at Rightway Healthcare supports members by providing clinical triage, proactive outreach, and care coordination. This registered nurse reports to the Sr. Clinical Manager and helps members navigate the healthcare system, educates them about care options, and connects them with trusted providers. The work aims to improve health outcomes while reducing unnecessary costs.
What you will do
- Perform clinical triage using evidence-based protocols to assess member health concerns and link them to suitable healthcare resources.
- Apply data analytics to identify high-risk members or those with care gaps, reaching out to support informed healthcare decisions.
- Engage in conversations with members to understand their health needs, preferences, and barriers, offering personalized guidance to improve outcomes.
- Identify obstacles that prevent access to preventive care or ongoing relationships with primary care providers.
- Assist members with scheduling appointments and referrals to ensure continuity in preventive care and chronic disease management.
- Deliver personalized clinical navigation, including education on conditions, care options, and coordination to help members follow care plans.
- Collaborate with the Rightway clinical lead to determine the best clinical options for complex member needs.
- Accurately document nursing assessments, member responses, and care guidance.
- Advocate for members throughout their healthcare journey.
