This location serves clients on a per-visit basis across Windsor and Orange counties in Vermont, as well as Grafton County in New Hampshire. While prior home care experience is preferred, it is not a requirement. As a Medical Social Worker, you will play a vital role within a multidisciplinary healthcare team, providing essential skilled nursing and palliative care that enables clients to receive the support they need to remain in the comfort of their homes. Key Responsibilities: Conduct home visits to clients within designated geographic areas. Assist the team in understanding the social and emotional aspects impacting our clients’ health. Perform clinical social evaluations and devise suitable interventions based on findings. Identify community resources to meet clients' and families' practical and environmental needs. Serve as an advocate to help clients and families navigate community systems. Engage in the development of comprehensive care plans alongside BAYADA's multidisciplinary team. Establish and maintain partnerships with public and private agencies for patient resources. Educate patients, families, or representatives on community resources that promote health, safety, and independence. Accurately document observations, interventions, and evaluations related to patient care using advanced technology.

This role involves performing emergency night visits to hospice clients in Windsor, Orange, and Grafton counties across Vermont and New Hampshire. Schedule: Rotating shifts, on-call from 5 PM to 8:30 AM, one week on followed by one week off. Enjoy full-time benefits while working only 26 weeks a year!
