Your Role:
We are expanding rapidly and need a dedicated and compassionate Nurse Practitioner to join our Advantmed provider network. In this role, you will conduct in-home wellness assessments for Medicare members and other at-risk populations, focusing on their overall health to ensure precise risk adjustment coding and improved value-based care. This 1099 (PRN) position offers flexibility for those managing various commitments.
Responsibilities:
In-Home Clinical Assessments:
- Travel to members’ residences to perform comprehensive health evaluations.
- Conduct Annual Wellness Visits and assessments for members with chronic conditions.
- Engage in medical history and physical examinations, review of systems, vital signs collection, medication review, and reconciliation.
- Administer preventive health screenings such as Hemoglobin A1C, Spirometry, Kidney Health Evaluations, and Diabetic Retinal Exams.
Patient Education & Engagement:
- Provide preventive health education and counseling to empower members in managing their health.
