As a Resource Nurse, you will engage with patients in their homes, providing comprehensive care ranging from high-acuity medical needs to basic health management. This enduring role allows for the development of meaningful, long-term relationships with the patients you serve, ensuring they receive the support they need over time.
In this position, you will advocate for individual patient needs, working collaboratively with Certified Nursing Assistants (CNA), Home Health Aides (HHA), Case Managers, and physicians to deliver the highest quality of care.
