As a Resource Nurse, you will engage with patients in their homes, providing comprehensive care ranging from high-acuity medical needs to basic health management. In this position, you will advocate for individual patient needs, working collaboratively with Certified Nursing Assistants (CNA), Home Health Aides (HHA), Case Managers, and physicians to deliver the highest quality of care.
