Your Role:
As a crucial member of our interdisciplinary team, you will work under the guidance of a Clinical Manager to coordinate all facets of patient care, ensuring personalized and compassionate support that enhances the quality of life.
Key Responsibilities:
- Conduct comprehensive, ongoing assessments of patients and their families
- Develop and oversee detailed care plans in collaboration with the care team
- Facilitate interdisciplinary case discussions and planning meetings
- Work closely with physicians, hospice staff, volunteers, and external organizations to provide optimal care
- Advocate for patients and families, addressing their physical, emotional, and psychosocial needs
