Responsibilities:Facilitate the mental and physical health care of a designated patient caseload. Conduct screenings and assessments for prevalent mental health and substance abuse disorders, enhancing patient engagement and follow-up care. Educate patients on common mental health issues and available treatment options. Consistently monitor treatment response and check in with patients (in-person or via phone) regarding clinical symptoms and any treatment side effects. Assist in psychotropic medication management through collaboration with behavioral health consultants and communication with primary care providers. Implement brief behavioral interventions utilizing evidence-based techniques, including motivational interviewing and problem-solving treatment. Provide or facilitate referrals to evidence-based psychosocial treatments as clinically necessary. Engage in regular case consultations with psychiatric consultants to review new patients and those not showing expected improvements.
