Compassionate Social Worker

Key Responsibilities:Work as part of an interdisciplinary team, reporting to a Clinical Manager, while planning and providing supportive services to patients and their families during challenging times. Identify and address the psychosocial needs of patients and families through direct service and consultations in alignment with the hospice care plan. Qualifications:Master's degree in Social Work or a related field (eligibility for social worker’s license accepted). Current Massachusetts social work licensure (LICSW preferred; LCSW accepted with relevant experience). At least three years of counseling experience preferred. Experience providing support to terminally ill patients and their families, with a solid understanding of the healthcare delivery system and community resources. Training and experience in leading support groups, along with skills in family psychosocial assessment. Proven ability to assess and respond effectively to the needs of patients and families in diverse settings. Thorough understanding of hospice philosophy and principles. Ability to work autonomously, adapt flexibly to changing circumstances, and collaborate effectively within an interdisciplinary team.
