We are looking for an exceptional Licensed Practical Nurse! Previous home health experience is highly preferred.

This is a full-time position with an expectation to complete 30 productivity points each week. Clinicians are compensated per point and have opportunities for additional earnings if they complete more than 30 points weekly.

Key Responsibilities:

As an LPN, you will provide comprehensive care in patients' homes, including maintaining accurate documentation through Homecare Homebase.

  • Comfortable with wound care management

  • Act as the patient's advocate, facilitating communication with physicians, referral sources, caregivers, and facility management to ensure coordinated care.

  • Administer medications and treatments as prescribed by physicians, including direct patient care such as infusion therapy, wound care, patient education, and disease management.

  • Participate in regular case conferences and in-services to enhance care quality.

  • Provide care to approximately 6 patients daily, Monday through Friday.

Qualifications:

  • Strong organizational and time management skills

  • Ability to multitask effectively and meet deadlines

  • Professional appearance and strong communication abilities

  • Valid Texas LPN license

  • Minimum of three years of clinical nursing experience

  • Valid Texas driver's license

  • CPR certification required

  • Experience with healthcare software is a plus
