This location serves clients on a per-visit basis across Windsor and Orange counties in Vermont, as well as Grafton County in New Hampshire. While prior home care experience is preferred, it is not a requirement. As a Medical Social Worker, you will play a vital role within a multidisciplinary healthcare team, providing essential skilled nursing and palliative care that enables clients to receive the support they need to remain in the comfort of their homes. Key Responsibilities: Conduct home visits to clients within designated geographic areas. Assist the team in understanding the social and emotional aspects impacting our clients’ health.
