    • Earn $75.00 per productivity point.

    • Annual salary ranging from $100,000 to $130,000.

As a Physical Therapist, you play a vital role in achieving this mission by restoring patient functionality, ensuring safety at home, and delivering measurable health outcomes.

Your Responsibilities:

  • Case Management: Take ownership of your caseload, ensuring accountability for each patient's therapy plan and outcomes.

  • Conduct thorough in-home assessments and provide skilled physical therapy sessions.

  • Develop and implement comprehensive care plans focusing on gait, balance, transfers, strength, functional mobility, home safety, and fall prevention.

  • Establish measurable goals, monitor patient progress, and modify treatments as necessary to ensure continued improvement.

  • Facilitate multidisciplinary communication regarding patient condition changes, treatment orders, and coordination of care.

  • Oversee treatment outcomes directly, including supervision of Physical Therapy Assistants (PTAs) when applicable.

  • Guide patients and their families in executing effective home exercise programs.

  • Ensure timely and accurate documentation in Homecare Homebase (HCHB) for defensible and efficient record-keeping.
