Earn $75.00 per productivity point.
Annual salary ranging from $100,000 to $130,000.

As a Physical Therapist, you play a vital role in achieving this mission by restoring patient functionality, ensuring safety at home, and delivering measurable health outcomes.
We leverage advanced technology to empower our clinicians in the field. Our innovative charting system minimizes documentation workload, while our upcoming scheduling tool facilitates efficient visit coordination, allowing you to focus on patient care.
Your Responsibilities:
Case Management: Take ownership of your caseload, ensuring accountability for each patient's therapy plan and outcomes.
Conduct thorough in-home assessments and provide skilled physical therapy sessions.
Develop and implement comprehensive care plans focusing on gait, balance, transfers, strength, functional mobility, home safety, and fall prevention.
Establish measurable goals, monitor patient progress, and modify treatments as necessary to ensure continued improvement.
Facilitate multidisciplinary communication regarding patient condition changes, treatment orders, and coordination of care.
Oversee treatment outcomes directly, including supervision of Physical Therapy Assistants (PTAs) when applicable.
Guide patients and their families in executing effective home exercise programs.
Ensure timely and accurate documentation in Homecare Homebase (HCHB) for defensible and efficient record-keeping.
