YOUR ROLE
As a Community Team Member, you will collaborate with residents to identify the root causes of their challenges and assist them in discovering effective solutions. Your goal is to foster independence by empowering their strengths. You will work to prevent social isolation and disadvantage while enhancing social integration and participation. As a passionate connector, you will engage with the right individuals from your network, including chain partners and the residents' own social networks. You provide targeted support, whether it be at the resident's home, in the office, or at another suitable location.
You will assist a diverse range of Amsterdammers, including those with mild intellectual disabilities, autism, acquired brain injuries, and mental health issues. You will act as a sparring partner for colleagues in one or more areas of expertise, drawing from both practical experience and theoretical knowledge in complex and crisis situations. Furthermore, you will assume the role of case manager for complex cases involving significant and multiple issues. You will make decisions regarding additional WMO provisions if the team's resources are insufficient to resolve the case. With your experience, this is well within your capabilities, and you find it enjoyable! You understand how to manage these situations with empathy while maintaining a professional distance. You won’t be alone in this; within the Community Team Southeast, you will collaborate with peers, including individuals with lived experience and volunteers in a multidisciplinary team. Each Community Team Member has their own specialization while also possessing fundamental knowledge across other expertise areas within the team. Together, you will make a significant difference in the lives of many Amsterdammers.
