The Bilingual Advanced Medical Assistant at Luminis Health supports the Maryland Primary Care Program (MDPCP) in Annapolis, MD. This role blends clinical and administrative tasks to coordinate patient care, provide education, and address barriers that can prevent effective treatment. The position plays a key part in reducing hospital readmissions and avoiding unnecessary emergency room visits by working closely with the Advanced Primary Care Team.
Main responsibilities
- Monitor and confirm patient assignments to the practice and care teams (empanelment).
- Coordinate proactive care for high-risk patients, focusing on relationship-building and ongoing support.
- Participate in pre-visit planning for Annual Wellness Visits with the LPN team or provider.
- Educate patients about their care plans and strategies for improving health.
- Manage a patient panel, develop relationships, and follow up to encourage adherence to provider recommendations.
- Use Value-Based Payor portals to attest to metrics and review care gap reports.
- Contact patients promptly after Transitional Care Management or Emergency Department visits.
- Assist patients in setting SMART goals for self-management and communicate abnormal findings to providers.
