Evaluate and document patient status, initiating and modifying care plans as required, and reassessing patient and family needs during after-hours visits. Develop individualized care plans with clearly defined goals, integrating palliative nursing actions while involving patients and families in the planning process. Implement preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed by physicians. Assist patients with daily living activities, promoting self-sufficiency and optimal comfort care.
