Position Overview: We are seeking a dedicated and detail-oriented Recovery Care Coordinator III to join our team in Fort Carson, CO. This is a full-time position that focuses on non-clinical case management for Recovering Service Members (RSMs) and their families, ensuring a comprehensive approach to their recovery journey. Key Responsibilities: Act as a non-clinical case manager for RSMs, advocating for their needs and managing their individual cases through the Comprehensive Recovery Plan (CRP).Develop and maintain ongoing Comprehensive Recovery Plans (CRP) tailored to the unique needs of each service member.Serve as a liaison between RSMs, their families, caregivers, and unit leadership to facilitate effective communication and support.Provide education on available benefits, coordinate care transitions, and monitor progress in the WCP Database.Transport RSMs as necessary, with prior approval from designated authorities.Build trust and rapport with stakeholders involved in the warrior care process.Employ strong time management skills to meet all mandated milestone markers as outlined in USSOCOM Directives and WCP Policies and Procedures.Collaborate with community partners, including veteran organizations and family readiness centers, to establish resources that support RSMs and their families.Conduct briefings with RSMs' unit leadership regarding the current status of CRPs and recommend courses of action for RSMs.Perform general office and administrative tasks to support program operations.
