Role Overview Wider Circle is seeking a Telehealth Physician to support our Wider Care+ pilot program, serving older adults and individuals with complex clinical needs. This fully remote position is based in Philadelphia, Pennsylvania, and focuses on providing community-based, relationship-driven care. Our approach brings together physicians, Community Health Workers, Licensed Clinical Social Workers, and care navigators to close gaps that traditional healthcare often misses. What You Will Do Conduct initial telehealth assessments for new program members (typically 10-15 minutes each). Review and analyze medical and social histories to identify clinical priorities and risks. Create clear and actionable clinical plans for Community Health Workers, LCSWs, and care navigators to follow. Document assessments, findings, and recommendations accurately in the EMR to support ongoing care. Provide clinical guidance on escalation pathways and program processes throughout the pilot. Build rapid rapport and trust with members and their caregivers during telehealth visits. Engage in Person-Centered Planning processes. Work with clients, families, Case Managers, DSPs, and other teams to implement and assess client scheduling requirements. Participate in client ISP meetings. Maintain ongoing communication with Case Managers/Care Coordinators and other stakeholders. Qualifications: A Bachelor’s degree is required. Minimum of 1 year of experience with the I/DD population is preferred. Previous experience in a medical office or home care setting is a plus. Prior supervisory experience is advantageous. Proven track record of successfully taking on additional responsibilities and achieving goals. A strong desire to grow and advance within the organization. Excellent computer skills.
