Qualifications
Responsibilities:Effectively manage patient census with a focus on closing clinical and non-clinical care gaps. Identify and coordinate community resources that enhance patient care. Educate patients on managing chronic conditions and improve health literacy. Oversee medication management, including identifying potential issues, reconciling medications, ensuring adherence, and coordinating refills. Facilitate timely service delivery to patients, including Home Health, DME, Home Infusion, and other essential services. Address patient inquiries and foster open communication to better understand their needs. Assist with referrals and appointment scheduling. Requirements:Active Multi-State/Compact License (LPN) (NLC) (LVN)Proficient in Microsoft Office SuiteExceptional written and verbal communication skillsPlusses:Experience with Epic systemsBilingual candidates are encouraged to applyPossession of additional single-state LPN license(s)Remote Requirements:Access to high-speed Wi-FiA home office or HIPAA-compliant workspacePhysical Requirements:This is a sedentary position requiring prolonged periods of sitting.
