CareHarmony is seeking a Licensed Practical Nurse (LPN) based in Massachusetts to join our remote care coordination team. This position focuses on supporting patients with chronic conditions by identifying resources and coordinating their care. The work is fully remote, allowing you to contribute from home while helping patients navigate their healthcare journeys. Role overview This role centers on value-driven care management. As an LPN, you will use your clinical experience to help patients access the resources they need and ensure their care plans are followed. Attention to detail and a proactive approach are important, as you will play a key part in improving patient outcomes. Requirements Active LPN license At least three years of direct patient care experience Strong organizational skills and self-motivation Comfort working in a changing, team-oriented setting What you will do Coordinate care for patients with chronic health conditions Identify and connect patients with appropriate resources Support patients throughout their care journey Benefits Fully remote role based in Massachusetts Full-time schedule: Monday to Friday, day shift No weekend work; minimal on-call (about once per year) Opportunities for career advancement within CareHarmony Hourly rate: $23/hr
