Your Role:
As a crucial member of our interdisciplinary team, you will work under the guidance of a Clinical Manager to coordinate all aspects of patient care, ensuring personalized and compassionate support that enhances quality of life.
Key Responsibilities:
- Conduct thorough, holistic assessments of patients and their families
- Develop and oversee individualized care plans in collaboration with the care team
- Facilitate interdisciplinary case discussions and care planning meetings
- Collaborate with physicians, hospice personnel, volunteers, and external organizations to provide optimal support
- Advocate for patients and families, addressing their physical, emotional, and psychosocial needs
What You Bring:
- Active Massachusetts RN license in good standing
