About Hopscotch Primary Care
At Hopscotch Primary Care, we are dedicated to ensuring that high-quality healthcare is accessible to everyone in our communities. With nearly 20% of Americans residing in rural areas and only 11% of physicians serving them, our mission is to revolutionize healthcare in rural America. We deliver tailored primary care that meets the unique needs of our patients through a comprehensive care model and a dedicated care team, offering services at our clinics and beyond, while providing essential resources to those who need them most.
Our patients, alongside the care teams committed to serving them, are at the core of everything we do. At Hopscotch, we adopt a collaborative approach to address patient needs and deliver the best possible care. Our aim is to provide care that each of us would want for ourselves or our loved ones, in the most appropriate setting and at the ideal time.
Currently, we are proud to serve thousands of patients within our value-based care framework, and this number continues to grow daily. If you are passionate about making a meaningful impact in healthcare, we would love to connect with you.
