Please Note: All positions at 21HHS require field visits to patients' homes.
Your Role: As a Full-Time Physical Therapist Case Manager in the Castro Valley area, you will collaborate with other clinicians to deliver skilled services aimed at assessing and addressing the needs of individuals in their homes, empowering them to live safely and productively.
You will focus on restoring function and minimizing disability resulting from illness or injury, helping clients achieve their maximum potential. Responsibilities include evaluating and treating patients according to prescribed therapy programs and supervising Physical Therapy Assistants (PTAs) in compliance with agency and regulatory standards.
