Job Summary
We are looking for a warm and empathetic Gastroenterology Nurse Practitioner/Physician Assistant to join our dynamic team. Responsibilities
- Deliver comprehensive healthcare services and coordinate patient care effectively.
- Assess and treat patients with gastrointestinal conditions.
- Collaborate closely with a multidisciplinary team of healthcare providers.
- Evaluate, diagnose, and address both physical and mental health of residents.
- Conduct comprehensive patient assessments, including medical history and physical examinations.
- Order and interpret diagnostic tests appropriately.
- Develop accurate diagnoses and formulate effective treatment plans.
- Prescribe medications and administer necessary treatments.
- Provide counseling and support to residents and their families.
- Work with the interdisciplinary team to ensure seamless care coordination.
- Manage chronic health conditions and monitor patient progress.
- Adjust care plans as needed based on patient requirements.
- Demonstrate a commitment to person-centered care.
- Create a safe and nurturing environment for elderly residents in nursing homes.
Requirements
- Minimum 6 months of experience in long-term care or a skilled nursing facility.
- Active and unrestricted Nurse Practitioner or Physician Assistant license.
