We are currently seeking a compassionate and experienced Hospice Patient Care Manager / Clinical Manager (RN) to lead our clinical operations. In this pivotal role, you will oversee interdisciplinary teams, ensuring the highest standards of patient care while maintaining compliance and fostering collaboration. Your hands-on support will be crucial in enhancing patient experiences.

Key Responsibilities

  • Take on a management position with clinical and operational responsibilities for your team.
  • Act as a liaison between the Director of Patient Care Services, clinical staff, patients, families, and the community.
  • Manage scheduling, workflow, and compliance for clinical associates.
  • Provide direct patient care in the absence of clinical staff members.
  • Ensure compliance with hospice policies, state and federal regulations, and Silverado standards.
  • Promote quality improvement initiatives and interdisciplinary team collaboration.
