About the Position
Internal Title: ECM Clinical Manager
As the ECM Clinical Manager, you will play a pivotal role in overseeing clinical operations and managing a team of ECM Lead Care Managers to address the diverse medical, behavioral health, and social needs of clients. This position involves direct communication with Lead Care Managers, and occasionally with patients, through telephone calls, telehealth sessions, and Interdisciplinary Care Team meetings. You will ensure medication management and adherence, facilitate appropriate follow-ups with Primary Care Providers, Behavioral Health Providers, and specialists, and provide guidance in crisis situations. Collaborating with the broader care team, you will advocate for clients, ensure they receive necessary services, and track progress towards their individualized care plans. Additionally, you will monitor team performance, coach Lead Care Managers, and address performance issues as needed.
This is a hybrid role requiring travel within the Los Angeles County area, approximately 2-3 times per month.

Key Responsibilities
Hybrid responsibilities performed using HIPAA-compliant tools:
Supervise a team of 10-15 Lead Care Managers, both in-field and remotely, ensuring the delivery of high-quality care management services. The number of direct reports may vary based on operational needs.
Oversee the development of Individual Care Plans and assess the suitability of client needs for ECM Lead Care Manager management; serve as an escalation point when necessary.
Motivate the team to effectively achieve key performance indicators (KPIs).
Collaborate with the Director and other stakeholders to enhance care strategies and outcomes.
