Role Overview:
As a Case Manager, you will provide critical support to 6-10 households escaping domestic violence, sexual violence, stalking, or human trafficking. Your role will involve crisis intervention and tailored safety planning for individuals in immediate danger. You will assess client needs upon entry, connect them to essential services, and advocate for comprehensive care that respects their autonomy and goals.
Key Responsibilities:
- Deliver crisis intervention and create personalized safety plans.
- Guide victims in filing protective orders and completing necessary affidavits.
- Accompany clients to court and various appointments, including attorney meetings and hearings.
- Conduct thorough assessments of clients' physical, mental health, social needs, employability, educational requirements, and overall quality of life.
- Coordinate care among mental health professionals, service providers, advocates, and legal entities.
- Oversee children's services, including education and daycare, programming, and counseling.
- Regularly meet with clients to provide intensive case management and advocacy.
- Maintain accurate client records and documentation following agency protocols and best practices.
- Report client information accurately into designated program data systems.
- Ensure timely dissemination of client information to relevant staff members.
