JOB SUMMARY
We are seeking a dedicated Certified Diabetes Care and Education Specialist to join our team at Stanford Medicine Children's Health. In this role, you will provide comprehensive diabetes education services for our Diabetes and Obstetric Programs, collaborating closely with clinical care teams to implement effective diabetes management and education protocols in both inpatient and outpatient settings. You will lead diabetes education classes and support group services to empower patients and their families.
ESSENTIAL FUNCTIONS

Your responsibilities will include:
- Delivering diabetes self-management education (DSME) for newly diagnosed patients and their families.
- Facilitating outpatient diabetes classes as part of the Stanford Children's diabetes education initiative.
- Providing recommendations for diabetes management supplies and devices, including insulin pumps and blood glucose monitors, while offering ongoing education to patients and their families.
- Assisting patients in the use of insulin pumps and continuous glucose monitors, as well as teaching proper injection techniques with insulin pens or syringes.
- Maintaining clear communication with patients and families regarding care plans through in-clinic discussions, MyChart, emails, and follow-up phone calls.
- Collaborating with medical and nursing staff for effective discharge planning of diabetic patients and other interventions as necessary.
- Conducting in-service training and presentations on pediatric diabetes care for hospital staff and external organizations.
- Assisting in the prior authorization process for diabetes medications and devices when necessary.
- Facilitating the renewal of diabetes medication prescriptions through electronic medical records.
- Monitoring and documenting patient progress toward diabetes behavioral goals, ensuring effective communication with the healthcare team.
