As a Clinical Pharmacist, you will play a pivotal role in identifying, preventing, and resolving medication-related issues. You will provide expert information to healthcare professionals, patients, and the community, ensuring that the clinical and drug distribution components of pharmacy services operate cohesively to deliver patient-centered care.

Key Responsibilities:
Operational Duties
- Oversee formulary management and support Pharmacy & Therapeutics (P&T) Committees.
- Prepare evidence-based monographs and class reviews for P&T Committees.
- Coordinate and participate in Medication Use Evaluations.
- Contribute to developing clinical guidelines, policies, and protocols to enhance medication utilization.
- Educate healthcare professionals through various communication channels, including newsletters and the online formulary.
- Conduct evidence-based evaluations of medical literature.
- Precept and train pharmacy students and residents in experiential settings.
Quality Excellence
- Enhance patient care outcomes through formulary management and medication use policy development.
- Identify high-cost, high-volume, and high-risk drugs, monitoring their appropriate use and optimizing medication strategies.
- Conduct routine audits to assess formulary compliance and evaluate the impact of institutional decisions and guidelines.
- Participate in quality initiatives based on departmental or organizational needs.
- Support the planning, design, implementation, evaluation, and maintenance of medication use processes for improvement initiatives.
Clinical Responsibilities
- Interpret medication orders to ensure patient safety and optimal therapeutic outcomes.
