Clinical Pharmacist
Job Purpose:
The Clinical Pharmacist plays a vital role in enhancing patient care by fostering strong relationships with patients and healthcare providers. This position focuses on improving medication adherence and advocating for evidence-based treatment methodologies.
Key Responsibilities:
- Analyze clinician notes and patient assessments to gather necessary clinical data for prior authorization and care plan recommendations.
- Design and update disease-specific clinical protocols aimed at ensuring optimal medication adherence and cost-effective care.
- Continuously monitor patient adherence using various data sources, with an emphasis on oncology and specialty diseases.
- Conduct evaluations of electronic medical records to identify drug interactions, medication selections, and other critical factors that affect patient outcomes.
- Advise on alternative therapeutic regimens and disease-specific treatments aligned with established guidelines.
- Monitor therapy for side effects and medication-related issues, providing recommendations for adjustments to prescribers as necessary.
- Manage medication therapy, including reviewing medication regimens and advising clinicians on best practices.
- Engage in clinical interventions such as comprehensive consultations, motivational coaching, medication synchronization, and managing transitions of care.
- Track productivity and report weekly statistics to support growth initiatives.
- Oversee the daily activities of support staff, ensuring accuracy in documentation and compliance with protocols.
- Identify and address insurance-related issues, facilitating access to financial assistance when applicable.
- Supervise communication among support personnel in compliance with HIPAA regulations.
- Maintain all necessary registrations and certifications as a remote pharmacist.
