As a Community Coach, you will empower residents in need of support, guiding them through various life challenges, including independent living, daily activities, and sensitive topics such as grief, finances, relationships, and divorce.
Your role involves listening empathetically and providing not just guidance, but also encouragement for residents to take initiative where possible.
Residents approach us with diverse inquiries, and you will assist them with challenges related to independent living, dementia care, daily activities, informal caregiving, stress management, financial advice, loneliness, and domestic violence.
A glimpse into your day:
Upon contact from a resident, you will engage in a conversation with the host, followed by a comprehensive living conversation with the case manager, during which the resident's support goals will be established.
Once the living conversation concludes, you will take charge of the case as a Community Coach (HBO), tackling complex situations involving multiple issues, unique circumstances, or challenging behaviors. This role demands strong theoretical knowledge, critical thinking, independent analysis, and evaluation skills, all while following a plan-do-check-act cycle. You will also serve as a resource and sounding board for MBO community coaches.
Collaboratively with an MBO community coach, you will initiate a 1Household1Plan1Coordinator (1H1P1R) meeting, where you will sit down with the client, their social network, and relevant professionals to co-create a plan centered on the resident's aspirations for change.
Your primary objective is to offer and initiate the necessary support while also encouraging residents to become active participants in their journey toward empowerment. You will listen, assess needs and capabilities, provide insights and advice, facilitate connections, and inspire action.
Most residents will be seen bi-weekly, with some on a weekly basis. Together with your colleagues, you will monitor progress and schedule evaluation moments.
In addition to individual support, you will actively participate in community analyses and the development of local services, helping to establish and facilitate daily activities and group events in the neighborhood.
