About the role
As a Community Liaison, you will be instrumental in cultivating and sustaining partnerships within the community. The successful candidate will strategize and implement community outreach initiatives. This role involves identifying, operationalizing, and assessing the success of community partnerships focused on social determinants of health. Collaboration with the Enhanced Care Management team at Vynca will be essential. The ideal candidate is self-driven, enthusiastic, compassionate, and possesses a solid background in community support and patient engagement. This role is field-based and requires travel throughout Orange County. Candidates must reside within 20 miles of the assigned territory due to travel frequency.
Key Responsibilities
Establish and nurture relationships with community stakeholders, including resource centers, healthcare facilities, and advocacy organizations.
Facilitate referrals into Vynca care programs, specifically Enhanced Care Management.
Continually enhance knowledge of community services and resources; maintain regular communication with agencies serving vulnerable populations.
Analyze local market dynamics and leverage agency strengths.
Craft and implement strategic marketing initiatives.
Identify and articulate cultural considerations impacting our members.
Monitor sales activities and deliver presentations.
Assist with community events while upholding superior customer service standards.
Act as a liaison between patients and healthcare practices.
Strengthen referral partnerships through proactive outreach.
