Position Overview: As a Data Engineer, you will collaborate with the senior care management team to facilitate care management activities. You'll ensure that healthcare services are effectively delivered to our members and that their satisfaction and continuity of care are prioritized. This role involves acting as a liaison between care managers, providers, and members, engaging with them through phone outreach and home visits.
- Conduct outreach to members via phone or home visits to discuss their care/service plans, next steps, resources, and ongoing education.
- Coordinate care activities per the care/service plan, collaborating with healthcare providers and community partners.
- Address member and provider inquiries related to care plans and service requests.
- Develop an in-depth understanding of care management services to respond to complex issues.
- Communicate effectively with care managers and practitioners to facilitate member services.
- Perform assessments for members with complex needs and document care requirements.
- Maintain member records in compliance with state and regulatory standards.
- Support the care management team in triaging and escalating requests as necessary.
- Adhere to practice standards and policies compliant with contractual and regulatory obligations.
- Identify member needs and make appropriate referrals to Care Managers and community organizations.
- Educate members about available benefits and resources.
- Assist with training and development as needed.
- Perform other assigned duties and comply with all policies and standards.
