AHRC NYC is on the lookout for compassionate and dedicated Direct Care Workers (DCW) to provide essential support to individuals with developmental disabilities under the age of 21 within New York State's foster care system.
Compensation: $20 per hour
ROLE OVERVIEW:
The Direct Care Worker (DCW) will play a crucial role in offering personalized support services to young individuals with developmental disabilities in New York’s foster care system. Training and support services are tailored to meet each individual's unique needs and are delivered in their home and community settings. The DCW will collaborate closely with their supervisor to create effective service plans while ensuring adherence to HCBS Waiver regulations and the agency's guiding principles and policies.
KEY RESPONSIBILITIES
▪ Assist individuals with disabilities in leading self-directed lives and actively participating in their communities.
▪ Foster behaviors that promote inclusion and community engagement.
▪ Deliver direct training and support in both home and community environments.
▪ Empower individuals and their families by providing essential information for informed decision-making.
▪ Enhance self-esteem and develop assertiveness in individuals.
▪ Utilize formal and informal approaches to understand the preferences and desires of individuals.
▪ Help individuals identify and leverage available supports from their communities and social networks, cultivating relationships with friends, family, and community members.
▪ Collaborate professionally with individuals and stakeholders to design and implement personalized support strategies that build on individual strengths and choices.
▪ Assist individuals in managing daily life aspects by providing skill-building support focused on their capabilities.
▪ Seek opportunities for professional growth for individuals and share valuable resources.
▪ Facilitate effective communication between team members and external agencies to enhance community opportunities.
▪ Participate in special case conferences and relevant meetings as required.
▪ Understand the challenges faced by individuals in pursuing self-directed lives and assist them in overcoming these obstacles through advocacy and self-advocacy facilitation.
▪ Employ safe and appropriate strategies for crisis prevention, intervention, and follow-up.
▪ Complete essential program documentation efficiently and promptly.
▪ Ensure program compliance with HCBS Waiver regulations and the agency's philosophy and policies.
▪ Carry out additional duties as assigned.
