Role: As a Direct Support Professional, you will play a vital role in enhancing the quality of life for individuals in both residential and community settings. Your primary responsibility will be to offer compassionate support while empowering individuals to make informed choices in their daily lives.
Major Duties and Responsibilities:
- Execute Person-Centered Support Plans tailored to the unique needs of each individual.
- Assist with essential daily living skills, which may include meal preparation, personal hygiene, toileting assistance, housekeeping, and engagement in work and leisure activities.
- Administer medications and necessary treatments as prescribed.
- Maintain accurate documentation as required throughout your responsibilities.
- Communicate any incidents affecting the well-being of individuals to the Program Manager, Case Manager, or Wellness Liaison.
- Attend and participate in all required meetings.
- If authorized, transport individuals using agency vehicles.
- Perform other duties as assigned by management.

Knowledge and Skills:
Experience: Previous direct care experience is preferred but not mandatory.
Education: A high school diploma or equivalent is preferred.
Physical Requirements: Must be able to lift, push, and/or pull a minimum of 50 pounds, stand for extended periods, bend, push wheelchairs, and provide physical interventions as necessary. The noise level may vary depending on assigned duties.
Due to the nature of care provided, personal protective equipment (PPE) may be required, and will be provided by Disability Supports to ensure a safe working environment.
