Position Summary
As the Engagement Manager, you will guide provider clients through the successful implementation and continuous enhancement of Advanced Primary Care Models (APCM) and Chronic Care Management (CCM) programs. Your role will involve managing client relationships, supervising program delivery alongside internal teams and our clients, and ensuring that our initiatives yield significant clinical and financial results.
The ideal candidate will possess over 5 years of experience in healthcare client management, value-based care operations, or care management program delivery, with a strong ability to coordinate among multiple stakeholders, including external vendors and clinical service partners.
Key Responsibilities
Client Engagement & Relationship Management
- Act as the primary liaison for assigned clients, overseeing daily communication and managing expectations.
- Develop and maintain strong, trusting relationships with clinical, operational, and executive stakeholders.
- Facilitate regular meetings with service provider partners focused on assessing performance, addressing barriers, and pursuing strategic objectives.
- Proactively seek out opportunities for program enhancements, workflow improvements, or service extensions.
