Compensation:
Approximately $100,000 to $133,000 annually, with payment of $64.10 per productivity point.

This is a full-time role, requiring the completion of 30 productivity points weekly, with opportunities for additional earnings based on performance.

Key Responsibilities:
- Conduct comprehensive assessments in patients' homes, documenting skilled care through Homecare Homebase.
- Perform detailed evaluations of the patient and home environment, including clinical documentation, visit notes, and care plan development, while maintaining effective communication with physicians, team members, patients, and families.
- Administer wound care and infusion therapies, advocating for patients and coordinating with various stakeholders for optimal care.
- Dispense medications and treatments as prescribed, providing direct patient care along with education on disease management.
- Supervise nursing team members, including LVNs and HHAs, ensuring quality clinical care.
- Engage in regular case conferences and in-service training.
