Role Overview
As a Family Support Services Coordinator, you will collaborate with management to conduct outreach to families in need of our services and their case managers. Your responsibilities will include processing new participants by collecting their information and organizing it for thorough review. In close partnership with the Senior Director of Family Support Services and the FSS Lead, you will ensure that our programs are well-organized and timely, allowing us to deliver outstanding support to the families we serve.
Work Location
This is a hybrid role, allowing you to work from our office in Saint Paul, MN, with flexibility to work from home.
Key Responsibilities
- Support marketing and recruitment initiatives to expand the Family Support Services program.
- Act as the primary contact for new program participants, explaining the Family Support Services offered by Accord.
- Coordinate and document initial meetings with new families.
- Manage documentation for participant accounts, ensuring accuracy and thorough data entry across multiple systems.
- Maintain regular communication with internal and external partners, participants, and their families regarding updates and service information.
- Provide person-centered support focused on compliance and quality service delivery.
- Identify and resolve issues, offering solutions to enhance processes and participant experiences.
- Prepare cost proposals and implementation plans tailored to each participant.
- Oversee the annual renewal process, including tracking Service Agreements with our providers.
- Ensure understanding and compliance with 245D licensing regulations.
- Conduct audits of caseloads and others’ as a Designated Coordinator.
- Perform additional duties as assigned.
