Key Responsibilities:
As a Field Case Manager, you will play a crucial role in ensuring the delivery of high-quality home health services. Your responsibilities will include:
- Timely completion of nursing assessment forms in accordance with agency policies.
- Supervision of Home Health Aides on a bi-weekly basis.
- Assessment of clients' physical, emotional, and developmental needs.
- Creation of individualized nursing care plans that accurately reflect clients' current needs prior to case conferences.
- Provision of health education for clients and their families.
- Accurate clinical documentation to showcase the delivery of safe home health care services.
Implementation of Best Practices:
- Conducting client assessments during every home visit.
- Effective communication of assessment findings to relevant stakeholders.
- Implementing nursing interventions to address client needs and changing conditions.
- Maintaining electronic documentation of clinical records for each home visit in compliance with agency protocols.
- Transcribing and executing the physician’s care plan promptly and accurately.
- Adhering to infection control and safety protocols as per agency guidelines.
- Monitoring clients to ensure adherence to prescribed care plans.
Conducting Specialized Procedures:
- Providing wound and PICC line care.
- Inserting and irrigating urinary catheters.
- Performing suctioning and cleaning of tracheal tubes.
- Drawing blood for laboratory analysis.
- Complying with all relevant policies and regulations of the home health agency.
