As a Clinical Registered Nurse (RN) II at Centra Health, you will play a vital role in delivering personalized patient care aimed at promoting and restoring health while adhering to established medical and nursing guidelines. Collaborate effectively with physicians and a multidisciplinary team to create, implement, and assess comprehensive care plans tailored to individual patient needs. You will manage a full patient assignment, utilizing your skills to plan, analyze, intervene, and set measurable goals for optimal patient outcomes. Ensuring a safe and supportive care environment, you will provide compassionate nursing services to both patients and their families in accordance with the guidelines set by the Virginia Board of Nursing.
Clinical - Assessment
- Perform comprehensive patient assessments, prioritizing data collection based on immediate needs within defined protocols.
- Conduct ongoing evaluations to adjust care based on patient conditions and established protocols.
- Integrate nursing interventions within a multidisciplinary care plan, aligning with standards of care and desired patient outcomes.
Clinical - Planning
- Develop and adapt individualized care plans that incorporate medical directives and contributions from various healthcare disciplines.
- Establish measurable patient goals in consultation with patients, families, and the healthcare team.
Clinical - Intervention
- Administer treatments as prescribed by healthcare providers promptly and accurately.
- Deliver therapeutic nursing interventions according to tailored care plans for patients and their families.
- Identify potential issues and respond appropriately to urgent or emergent patient needs.
- Initiate resuscitative measures as necessary.
- Take responsibility for managing care for a specialized patient population effectively.
- Provide care in a culturally sensitive manner that respects the autonomy, dignity, and rights of patients and families.
- Collaborate with interdisciplinary team members to ensure the appropriateness of patient care interventions.
Clinical - Evaluation
- Assess patient responses to care to evaluate the effectiveness of nursing interventions in relation to established goals.
- Make necessary adjustments to the interdisciplinary care plan based on evaluations and collaborate with specialty teams to enhance patient outcomes.
