Key Responsibilities:
Patient/Family and Community Engagement
Care Coordination
- Conducts comprehensive assessments for all assigned patients, ensuring thorough documentation.
- Communicates any changes in a patient's condition promptly and effectively.
- Utilizes appropriate nursing diagnoses and data from assessments to establish realistic outcomes and documents these goals within the care plan; modifies plans as necessary to meet patient objectives.
- Identifies and prioritizes patient needs/nursing diagnoses and documents accordingly.
- Collaborates with patients, families, and the healthcare team during care planning, documenting all interactions. Assesses patients for discharge planning and makes necessary referrals.
- Evaluates and documents patient responses to nursing interventions, adjusting the care plan as needed.
- Ensures proper discharge instructions and medication reconciliation, providing patients and families with details on portal usage and other key resources.
Patient Education, Ethics, and Diversity
- Supports the spiritual, cultural, and educational needs of patients and families during assessments, documenting their learning levels and readiness.
- Employs the teach-back method to enhance patient understanding of medication side effects and new medications.
- Utilizes and distributes Krames educational materials for clinical conditions.
- Incorporates cultural vision resources in care planning and delivery.
- Treats patients, families, and colleagues with respect and dignity, acknowledging the role of significant others in care plans.
- Adheres to ethical practice standards and calls for ethics consultations as needed.
Evidenced-Based Practice and Research
- Implements nursing interventions aligned with the established care plan based on evidence-based practices.
- Maintains and enhances the professional image and science of nursing.
