- The registered professional nurse is accountable for providing individualized, age-specific care for patients throughout their healthcare journey from admission to discharge. Responsibilities include delivering direct care to assigned patients, making ethical decisions, collecting and analyzing assessment data to determine nursing diagnoses, collaborating with patients, families, and healthcare teams, developing a care plan to achieve realistic outcomes, providing evidence-based education, and systematically evaluating nursing practice effectiveness.
Key Responsibilities:
- Assess and document patient status and care needs upon admission.
- Organize and maintain accurate and accessible health data.
- Identify and prioritize problems requiring immediate attention.
- Validate diagnoses in collaboration with patients, families, and healthcare teams.
- Document diagnoses to facilitate expected outcomes and care plans.
- Initiate and update the Problem List at admission, incorporating age-specific care principles.
- Ensure the individualized care plan is discussed and agreed upon with patients and families.
- Commence discharge planning at admission, consulting with the patient/family and healthcare team members.
- Identify and address patient teaching needs throughout the hospital stay.
- Facilitate discussions between patients, families, and healthcare team members as needed.
