Monday-Thursday 7:45 AM - 4:15 PM, Friday 8:30 AM - 5:00 PM
The Behavioral Health team is actively looking for a Licensed Registered Nurse (RN) who possesses either three years of nursing experience or one year of nursing experience accompanied by two years of behavioral health experience.
The RN Case Manager embodies the mission, vision, and values of Northwestern Medicine, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, as well as regulatory and accreditation standards.
This role involves planning and delivering patient care aimed at achieving optimal patient outcomes while employing a collaborative approach to assess, plan, implement, coordinate, monitor, and evaluate the services required to meet patients' ongoing healthcare needs.
Key Responsibilities:
- Screen and evaluate service requests.
- Intervene and manage acute crisis situations.
- Assess client needs, strengths, goals, and resources.
- Collaborate with an interdisciplinary team to review cases and formulate treatment plans.
- Create individualized treatment plans.
- Screen for tardive dyskinesia in clients prescribed phenothiazine medications.
- Update diagnoses in consultation with psychiatrists.
- Participate in managing the caseload of clients in the Rehabilitation Services Program.
- Facilitate medication groups and coordinate services with psychiatrists.
- Implement treatment using various therapeutic modalities including individual, family, group, or conjoint therapy.
- Consult with center staff on problem assessment and treatment planning.
- Serve as a liaison to other agencies when needed.
- Provide training for other providers and non-providers in service specialties.
- Meet quarterly with nursing staff to review medical procedures and issues.
- Consult program staff regarding patients' medical/physical issues.
- Assist in planning treatments to improve patients' medical/physical conditions.
- Administer approved medication when authorized by a physician.
- Monitor lab test results and collaborate with program staff to optimize patient care.
