Key Responsibilities:
Patient/Family and Community Engagement
Care Coordination
- Conduct comprehensive assessments for each assigned patient, ensuring accurate documentation.
- Promptly communicate any changes in the patient’s condition.
- Utilize appropriate nursing diagnoses and data to establish realistic outcomes, documenting measurable goals in the care plan; modify the plan as necessary to align with patient objectives.
- Identify and prioritize patient needs and nursing diagnoses for accurate documentation.
- Collaborate with patients, families, and interdisciplinary healthcare teams in planning care, documenting accordingly. Assess discharge planning needs and make suitable referrals.
- Evaluate and document patient responses to nursing interventions, adjusting the plan of care when needed.
- Ensure thorough discharge instructions and medication reconciliation, equipping patients/families with information about the patient portal and relevant resources.
Patient Education, Ethics, and Diversity
- Support the spiritual, cultural, and educational needs of patients and families by assessing their learning levels and readiness, documenting educational records accordingly.
- Employ the teach-back method to enhance patient education regarding medication side effects and new medications.
- Utilize and distribute Krames materials for patient education on clinical conditions.
- Incorporate cultural vision resources into care planning and delivery.
- Demonstrate respect and dignity towards patients, families, and staff.
- Recognize the significant contributions of patients’ loved ones as integral to the care plan.
- Adhere to ethical standards in practice and request ethics consultations as appropriate.
Evidenced-Based Practice (EBP) and Research
- Implement nursing interventions aligned with established care plans based on evidence-based practices.
- Maintain and enhance the professional image and scientific foundation of nursing while advancing professional nursing practice.
