About the Role:
The Health Navigator plays a crucial role in simplifying the often overwhelming transitions within the healthcare system for both patients and providers. Your primary focus will be to cultivate trusted relationships with patients and healthcare providers, assisting with various administrative, logistical, and patient-facing tasks. This allows physicians to concentrate on delivering exceptional medical care. Health Navigators are employed by Main Street Health but work directly within our partner clinic settings, collaborating closely with clinic teams. Responsibilities include engaging with patients through in-person meetings and phone calls, entering vital data into our systems, managing prescription refills with pharmacies, resolving insurance issues, and more. Our management and training teams will provide you with the necessary resources and training to excel in your responsibilities.
- Establish strong relationships with patients to enhance their care experience.
- Assist patients during the check-in process, accurately entering data into the Main Street software platform for effective tracking and reporting.
- Proactively reach out to patients to schedule appointments or remind them about medication pick-up.
- Coordinate and schedule necessary procedures, tests, and screenings.
- Identify and monitor any gaps in patient care to ensure comprehensive support.
- Maintain thorough records of patient interactions and communicate effectively with healthcare providers using electronic health records.
- Educate patients about their healthcare options, insurance benefits, and common medical conditions.
- Assist patients in accessing community resources.
- Help facilitate follow-up care for patients post-hospital discharge.
- Exhibit compassion, adaptability, attention to detail, patience, and a collaborative spirit with team members.
