About the Role:
The role of a Health Navigator is pivotal in assisting patients and providers navigate the complexities of our healthcare system. You will foster trusting relationships with patients, providing administrative and logistical support to allow physicians to concentrate on delivering exceptional medical care. As a Health Navigator, you will work directly in one of our partner clinics, engaging with patients, managing data, coordinating prescription refills, and resolving insurance issues among other responsibilities. Our management and training teams will equip you with the necessary tools and education to excel in this position.
- Build and maintain meaningful relationships with patients to support their healthcare journey.
- Assist patients during check-ins, accurately entering data into the Main Street software platform for tracking and reporting.
- Proactively contact patients to schedule appointments and remind them of medication pickups.
- Coordinate and schedule necessary procedures, tests, and screenings.
- Monitor and address significant gaps in patient care.
- Keep detailed records of patient interactions and communicate effectively with healthcare providers via electronic health records.
- Educate patients on their healthcare options, insurance benefits, and common medical conditions.
- Assist patients in accessing community resources.
- Facilitate follow-up care for patients post-hospital discharge.
- Exhibit empathy, adaptability, attention to detail, patience, and a collaborative spirit within the team.
