- $75 per standard PT visit
- $112.50 for PT evaluations

Role Overview:
As a Physical Therapist, you will conduct thorough assessments in patients' homes and complete skilled documentation using Homecare Homebase.
Assess patients and implement appropriate therapeutic interventions.
Create comprehensive care plans and coordinate multidisciplinary treatment.
Ensure timely clinical documentation and visit notes.
Maintain consistent communication with physicians, care team members, patients, and families.
Comfortably navigate various aspects of the home health environment.
Implement and uphold established physical therapy protocols.
Oversee patient treatment and outcomes directly or through PTA supervision.
Qualifications:
Must hold a current Texas Physical Therapist (PT) license.
At least one year of experience in physical therapy.
Familiarity with Homecare Homebase is beneficial, but not mandatory.
Prior experience in Home Health is an advantage, but not essential.
Possess a valid Texas driver's license.
Current CPR certification is required.
Employment Type:
