Reports To: Volunteer Coordinator
Position Summary:
The Hospice Volunteer plays a vital role in providing compassionate patient care and support services in accordance with their training and the policies of Impact Healthcare LLC.
Key Responsibilities:
1. Collaborate effectively as a member of an interdisciplinary team in various settings, including home care, respite care, inpatient support, bereavement, and administrative tasks.
2. Engage in comprehensive orientation programs.
3. Assist patients and their families/caregivers under the guidance of the Volunteer Coordinator and Case Manager, or as part of the administrative team.
4. Act as a substitute caregiver in the home, performing agreed-upon tasks such as homemaker chores, running errands, and providing respite care.
5. Maintain confidentiality while documenting activities as instructed.
6. Comply with all health requirements established by Crossroads Home Health & Hospice.
7. Ensure regular availability aligned with patient and family/caregiver needs.
8. Participate in volunteer support groups and educational in-services.
9. Foster open communication and report regularly to the Volunteer Coordinator.
10. Perform additional duties as assigned by the Volunteer Coordinator.
11. Contribute to quality assessment and performance improvement initiatives.
12. Attend interdisciplinary group meetings as appropriate.
Qualifications:
1. A mature individual who supports the hospice philosophy, is comfortable with their own spirituality, and is dedicated to serving others as a volunteer.
2. Successful completion of the hospice volunteer training and orientation program is required.
3. Previous volunteer experience is advantageous but not mandatory.
4. Ability to work effectively within an interdisciplinary group.
5. Must possess a valid driver's license and have access to an insured automobile in good working condition for home visits.
