As a key leader, you will be responsible for overseeing the implementation of integrated care programs that optimize service delivery through data-driven insights. Your efforts will significantly enhance care quality, align with patient, family, and care partner goals, and empower your team members to work to their full potential, including both direct and indirect care coordination.
- Lead an integrated palliative care team within a designated neighborhood, ensuring smooth coordination of services and continuity of care for optimal patient and employee experiences.
- Collaborate with your team to evaluate, plan, and coordinate with patients, caregivers, and other care team members to establish and refine care plans, respecting patients' wishes, including requests for Medical Assistance in Dying (MAID).
- Monitor and oversee indirect care coordination activities provided by both regulated and unregulated health professionals within your team.
- Support your team in delivering high-quality care, which includes pain and symptom management, advanced clinical skills, end-of-life care, interdisciplinary collaboration, cultural competence, emotional support, counseling, and advocacy.
- Ensure a seamless care approach for patients and caregivers, addressing their inquiries and building trusting relationships to understand their goals.
- Effectively manage relationships with key internal and external stakeholders, enhancing communication and collaboration.
- Address quality, safety, and risk issues by developing improvement plans and participating in quality-of-care reviews as needed.
