Position Title: Intensive Case Manager

Service Area/Programs: Intensive Case Management
Role Overview: As an Intensive Case Manager within the dedicated Intensive Case Management Team, you will play a pivotal role in delivering essential community rehabilitative services. This includes direct treatment, restorative interventions, and comprehensive case management aimed at helping clients manage psychiatric and substance use disorders effectively. Your mission will be to foster long-term symptom stability, enhance community living skills, and improve interpersonal relationships.
The ICM Team is committed to providing immediate crisis intervention and ensuring timely access to vital community resources. Key Responsibilities: Manage a caseload of 15-30 clients, ensuring a minimum of four face-to-face contacts per month in safe community settings. Maintain detailed progress notes and complete documentation in the electronic client database within 24-48 hours of service delivery. Meet a productivity requirement of no less than 55% monthly. Develop person-centered goals/objectives in Plans of Care in accordance with DBHDD guidelines. Facilitate follow-up and referrals as needed, connecting clients with community resources for housing and employment services. Act as a liaison among clients, physicians, and organizational components to maintain continuity of care. Participate in an on-call rotation and provide individual transportation as required.
